Royal Berkshire NHS Foundation Trust has received funding to either redevelop or relocate the hospital, which is currently based in Reading.
As part of the Building Berkshire Together programme, a survey has been launched to get opinions. It says challenges have been identified with redeveloping the hospital for 21st century care on the current site and alternative sites are being considered.
The trust is also holding a series of engagement events to give you the chance to share your views in person.
